The main beams on both sides are provided with two steel ropes, and the upper end of the steel rope is tied with four lifting ring screws anchored into the side beam of the building structure.

The unloading platform used on the construction site is processed and manufactured by Chongqing Shenghua company, and has been used in the main structure construction of 6 projects such as 1# business office building, 8 projects such as 9# business office building and 12 projects such as 1# business office building.
According to the special construction scheme, the stacking weight limit of the unloading platform is 1 ton, and there are no more than 283 90 cm scaffold tubes; Item (5) of Article 1 of the technical guidelines for safe operation of cantilevered steel platform on construction site stipulates that “there shall be no more than 2 operators on the cantilevered steel platform”..
At about 13:45, the rescue personnel arrived at the scene and confirmed that the three operators who fell from the unloading platform had no vital signs.
3、 Cause and nature of the accident after on-site investigation, the accident investigation team obtained relevant material evidence, documentary evidence and video data according to law, investigated and inquired relevant personnel, found out the cause of the accident and identified the nature of the accident.
Chongqing Shenghua company did not carry out thorough rectification, and neither Construction Engineering International Company nor shunjinsheng company took effective measures to urge the rectification.
The accident is a rollover accident of the unloading platform, in order to summarize the experience and lessons, do a good job in safety management and be responsible for yourself and others! PS: Rome wasn’t built in a day.
In addition, the disclosure scheme and acceptance form of the unloading platform are signed on behalf of others.
（1） The public security organ ruled out the suspicion of man-made intentional criminal crime in combination with the analysis of on-site investigation, autopsy, investigation and interrogation.

After investigation, two lifting ring screws on the east side of the unloading platform and one lifting ring screw on the west side were broken.
The unloading platform was not reprocessed.
The lifting ring screw on the west side was not broken, but the lifting ring was not close to the side beam of the building structure; The cantilever length of the unloading platform is 5.55m, turning to the East, and the protective fence falls to the ground; The anchor point of the steel wire rope lifting ring screw is 2.7m high from the main beam of the platform, and the included angle between the steel wire rope main rope and the horizontal steel beam is about 30 °.
On November 28, due to the jacking operation of the tower crane at the construction site of the incident project, the transfer and lifting of materials stacked on the unloading platform of the 3# business office building was suspended.
Accident process and emergency rescue (I) after the accident, at about 14:00 on November 26, 2020, Wang moupeng, the scaffolder leader of Chongqing Shenghua company, organized personnel to raise the unloading platform from the 9th floor of the 3# business office building to the 10th floor without scheme disclosure and safety technology disclosure and on-site supervision of full-time safety production management personnel.
At about 13:23 on November 28, 2020, at the construction site of 12 projects including 1# business office building of the original Banqiao phase III Project (excluding the third, fourth and fifth sections of the underground garage, hereinafter referred to as 12 projects including 1# business office building) located in zhaoquanying Town, Shunyi District, the unloading platform on the north side of the 10th floor of the 3# business office building overturned, resulting in three deaths and a direct economic loss of 4.8276 million yuan.
At about 12:00, Xiang Yiwu, the carpenter group leader of Chongqing Shenghua company, organized personnel to disassemble the scaffold tubes on the 10th floor of the 3# business office building when the tower crane was suspended, and stacked the removed scaffold tubes on the unloading platform.
A protective fence (1.5m high) is set at the edge of the unloading platform, and signboards such as load limit tonnage, stacking material details and platform operation procedures are hung on the inner side of the fence.
At about 13:23, the unloading platform overturned, and three operators and scaffold tubes on the platform fell to the ground.

According to the investigation, during the use of the unloading platform of the incident project, it was found that the included angle between the main rope of the steel wire rope and the horizontal steel beam was less than 45 °, the stacked materials were overloaded, the operators exceeded the limit, the inner side of the lifting ring was not close to the side beam of the building structure and other potential safety hazards.
It is a cantilevered steel platform.

（2） The emergency rescue situation was about 13:27, and the on-site personnel called the emergency telephone.
According to the verification of the public security organ, about 1100 scaffold tubes, with a total weight of about 3 tons and a falling height of about 30 meters, fell to the ground from the unloading platform.
